The following information is provided as a guide to the process of ordering a transcript of your University Extension course work. To download a PDF of the Transcript request form, please click here. (Acrobat reader is required.)
1. Mail or fax your request. No phone orders can be accepted. Allow up to two weeks for processing. Mail requests to:

Note: All credit card orders must include the full credit card number and expiration date. VISA, MasterCard, American Express, Diner's Club and NOVUS (includes Discover Card, BRAVO and Private Issue) are accepted.
2. Provide your full name and address and daytime phone number. Providing your Social Security Number (voluntary and kept confidential, for record identification only) will assist the process.
3. If different from your current name, please provide your name as it appears on our records.
4. Indicate whether you want the transcript sent now showing all graded courses to date or held until a specific course or courses have been graded. If requesting a hold for grades, please indicate the specific course(es) to hold for.
5. If your work was Concurrent Enrollment, please indicate if you wish to have any narrative evaluations sent, if they are on file at University Extension.
6. Indicate the number of copies you are requesting. Fees: $6 per copy; $6 for each additional copy ordered and sent at the same time. Payment is by credit card only.
7. Rush transcript service (one working day priority service) may be obtained by adding the $15 service fee to the fee(s) in #7 above and indicating that the request is for priority service. Please note that no processing takes place over weekends or regularly scheduled holidays.
8. Provide COMPLETE mailing address information, including ZIP code, for all recipients of the transcript.
9. SIGN YOUR REQUEST. No request will be honored without the signature of the student whose record is being requested.
