Withdrawal Request

Completion of this form indicates your desire to withdraw from a course for which you are enrolled through UCSC Extension. This request indicates that you are aware of and agree there is no refund available. All withdrawals must be requested seven days before the last day of class, and will result in a "W" grade notation. If you do not complete the course or this withdrawal request before the last day of class, your grade will default to an "F".
